LIABILITY RELEASE FORM
Intending to be legally bound hereby, the undersigned agrees and does hereby release from liability and to indemnify and hold harmless It’s All Volleyball LLC, and any of its employees or agents representing or related to It’s All Volleyball LLC, as regards to any activities involving practices, training, games, tournaments and clinics which includes any activities with in the building and on its premises. This release is for any and all liability for personal injuries (including death) and property losses or damage occasioned by, or in connection with any activity or accommodations for this and all events that occur at this location. The undersigned further agrees to abide by all the rules and regulations promulgated by It’s All Volleyball LLC, and/or its affiliates, groups and vendors throughout the year. We also reserve the right to use any photos or videos to promote the facility. By signing this waiver, you have read and agree to all COVID-19 Rules and Regulations set forth for this facility. I am aware that this sport may cause injury or death. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
